Your lungs work constantly, and there are practical ways to support their function. Whether you have a lung condition or simply want to breathe better, these evidence-based strategies can help.
Exercises that improve breathing
Breathing exercises are a cornerstone of pulmonary rehabilitation. They can reduce breathlessness, improve oxygen exchange, and help manage anxiety — whether you have a lung condition or not.[1]
Pursed lip breathing
One of the most effective and simplest breathing techniques, especially for people with COPD or exercise-induced breathlessness.[1]
- How to do it: Breathe in slowly through your nose for 2 seconds. Purse your lips as if you're about to whistle. Breathe out gently through your pursed lips for 4–6 seconds.
- Why it works: Creates gentle back-pressure that keeps airways open longer, reduces air trapping, and slows the breathing rate.[1]
- When to use it: During activity that causes breathlessness (climbing stairs, walking, bending), or whenever you feel short of breath.
- Practice: Start with 4–5 breaths at rest, several times a day. Use it naturally during daily activities as it becomes comfortable.
Diaphragmatic (belly) breathing
Retrains your diaphragm — the primary breathing muscle — to do more of the work, reducing effort from the neck and chest muscles.
- How to do it: Sit comfortably or lie on your back. Place one hand on your chest, the other on your belly. Breathe in slowly through your nose — your belly should rise while your chest stays relatively still. Breathe out slowly through pursed lips, letting your belly fall.
- Why it works: Engages the diaphragm more efficiently, increases tidal volume, and reduces the respiratory rate.[1]
- Practice: 5–10 minutes, 2–3 times daily. Can be combined with pursed lip breathing for added benefit.
- Note: Some patients with severe COPD or hyperinflation may find diaphragmatic breathing uncomfortable. Pursed lip breathing alone is effective and may be more practical for some people.
Box breathing (for stress and anxiety)
- How to do it: Breathe in for 4 seconds → hold for 4 seconds → breathe out for 4 seconds → hold for 4 seconds. Repeat 4–6 cycles.
- Why it works: Activates the parasympathetic nervous system, reducing heart rate and promoting calm. Widely used in high-stress environments (military, athletics, healthcare).
- When to use it: Before stressful situations, during anxiety, or as part of a wind-down routine before sleep.
4-7-8 breathing (for relaxation)
- How to do it: Breathe in through your nose for 4 seconds → hold for 7 seconds → exhale slowly through your mouth for 8 seconds. Repeat 3–4 cycles.
- Why it works: The extended exhalation promotes parasympathetic activation and relaxation. The breath hold allows more complete gas exchange.
- When to use it: Before sleep, during moments of anxiety, or as a quick reset during the day.
- Note: If holding for 7 seconds feels uncomfortable, start with shorter holds and build up gradually.

Exercise is one of the most effective ways to improve respiratory fitness, even for people with chronic lung disease.[2]
How exercise helps breathing
- Strengthens respiratory muscles: The diaphragm and intercostal muscles become more efficient with regular aerobic exercise.
- Improves oxygen delivery: Exercise increases cardiac output and enhances the body's ability to extract and use oxygen from each breath.
- Reduces breathlessness: As fitness improves, the same activities require less respiratory effort. What once made you winded becomes easier.
- Improves exercise tolerance: In pulmonary rehabilitation, structured exercise programs improve the 6-minute walk distance, reduce dyspnea, and improve quality of life — even in advanced lung disease.[2]
- Reduces inflammation: Regular moderate exercise has anti-inflammatory effects that may benefit airway health.
- Improves sleep: Exercise also improves sleep quality, which supports overnight respiratory function.[3]
Exercise recommendations
- General goal: 150 minutes per week of moderate aerobic activity (brisk walking, cycling, swimming) or 75 minutes of vigorous activity.
- Start where you are: If you're deconditioned or have lung disease, start with 5–10 minute walks and increase gradually.
- Walking: The simplest and most accessible exercise for lung health. Walk at a pace that makes you slightly breathless but still able to talk.
- Swimming: Warm, humid air is easier to breathe. Swimming strengthens respiratory muscles and improves cardiovascular fitness.
- Cycling: Low-impact, adjustable intensity. Stationary cycling is often used in pulmonary rehabilitation.
- Yoga and tai chi: Combine physical movement with breathing techniques. Evidence supports their role in improving lung function and reducing breathlessness.[2]
- Use your rescue inhaler before exercise if recommended by your clinician (common in exercise-induced asthma).
- Stop and rest if you experience chest pain, severe breathlessness, dizziness, or lightheadedness.
Protecting your lungs every day
Air quality and environment
- Don't smoke: Smoking is the leading preventable cause of lung disease. Quitting at any age improves lung function and reduces risk.
- Avoid secondhand smoke: Secondhand smoke increases the risk of asthma, COPD, and lung cancer in non-smokers.
- Monitor air quality: Check the Air Quality Index (AQI) at AirNow.gov. Limit outdoor exertion when AQI is unhealthy, especially if you have asthma or COPD.
- Reduce indoor pollutants: Use exhaust fans when cooking, avoid burning candles or incense frequently, keep humidity controlled to prevent mold, and service gas appliances regularly.
- Occupational exposures: Use appropriate respiratory protection if you work around dust, chemicals, fumes, or fibers.
Vaccines and prevention
- Annual flu vaccine: Influenza can cause serious complications in people with lung disease.
- Pneumococcal vaccines: Protect against pneumococcal pneumonia. Recommended for adults 65+, smokers, and those with chronic lung or heart disease.
- COVID-19 vaccine: Stay current with recommended doses.
- RSV vaccine: Now available for older adults and pregnant individuals to protect newborns.
- Hand hygiene: Regular handwashing reduces the risk of respiratory infections.
- Stay active: Deconditioning is one of the most common — and most reversible — causes of breathlessness.
When structured support helps
Pulmonary rehabilitation is a supervised program of exercise training, education, and self-management for people with chronic lung disease. It is one of the most effective interventions for improving quality of life in COPD and other respiratory conditions.[2]
- Typically involves 2–3 sessions per week for 6–12 weeks.
- Includes supervised aerobic exercise, strength training, breathing retraining, and education on disease management.
- Has been shown to improve exercise capacity (6-minute walk distance), reduce dyspnea, decrease hospitalizations, and improve quality of life.[2]
- Appropriate for COPD, pulmonary fibrosis, pulmonary hypertension, pre- and post-lung surgery, and other chronic respiratory conditions.
- Requires a physician referral. Ask your clinician if pulmonary rehabilitation is appropriate for you.
When mucus management matters
Airway clearance techniques help loosen and remove excess mucus from the lungs. They are essential for conditions like bronchiectasis, cystic fibrosis, COPD with chronic mucus, and neuromuscular disease.
- Techniques include huff cough, active cycle of breathing (ACBT), and postural drainage.
- Devices include oscillating PEP (Aerobika, Acapella, Flutter), high-frequency chest wall oscillation vests, and mechanical cough assist.
- Adjunct medications (hypertonic saline, bronchodilators) can enhance clearance.
